There are many reasons why your transaction could have failed.
The most common reason is due to NSF, other possibilities include the lack of email addresses, phone numbers, security questions, etc.

Most transactions will fail within 2-3 days of the transaction initiation but banking regulations allow for failures to occur even beyond this point.
